To apply for travel clearance para sa anak ninyong magta-travel lang ng mag-isa, you need na ihanda muna ang lahat ng kinakailangang documents bago kayo pumunta sa DSWD office.

Ang mga sumusunod na documents ang kailangan ninyong ihanda ayon sa DSWD.

1. Travel Clearance application form
You can get this application form sa DSWD office mismo. Kailangan nyong sulatan ang lahat ng kailangang information bago nyo ito submit.

2. Photocopy of Birth Certificate or Passport
Xerox nyo ang birth certificate at passport ng anak ninyo at isama ito sa isa-submit na mga documents. Meron time na maaaring manghingi pa ng additional documents ang DSWD para ma-ensure ang identity ng bata.

3. Written Consent of Parent
A written consent letter of the parent kung saan pinapayagan nyang mag-travel ang bata ng mag-isa papunta sa ibang bansa.

4. Document Certifying Child Custody
Kung ang parents ng bata ay kasal at buhay pa, a copy of the MARRIAGE CERTIFICATE is necessary. Kung ang bata ay nasa poder ng legal guardian or solo parent nya, a copy of CERITIFICATE OF LEGAL GUARDIANSHIP ang kailangan.

Kung ang bata naman ay illegitimate minor, kailangan nyong ipasa ang copy ng CENOMAR ninyo na makukuha sa NSO. Kung ang parents naman ay patay na, a photocopy of DEATH CERTIFICATE ay kailangan.

5. Pictures
Two colored passport size photos of the minor taken within the last six (6) months.
