If all of these are completed, then ihanda nyo ang mga documents sa baba upang makapag-apply kayo ng panibagong passport ng adopted child ninyo.

1. Amended NSO Birth Certificate (showing the new adoptive surname) in security paper authenticated by DFA Manila.

2. Old NSO Birth Certificate (using the original surname) in security paper.

3. Certificate of Registration of Adoption (issued by Manila Local Civil Registrar).

4. Photocopy of Authenticated Philippine Court Decree of Adoption.

5. Old passport.
