Passport renewal of married women want to use husband surname

Para sa kinasal na mga Pinay at gustong baguhin ang kanilang passport gamit ang surname ng kanilang asawa, these are the things you should do and the documents you should prepare.

1. Personal appearance of applicant.

2. Duly accomplished application form. (may be downloaded from Embassy website or fill-up online)

3. Passport for renewal.

5. Letter Pack 500 envelope.

6. Passport fee. (Note: Fees are payable in US Dollar or its equivalent in Japanese Yen as computed by our Cashier.)


For married women who wish to use their married surname, below are the additional documents that you should submit:

1. Report of Marriage or NSO Marriage Certificate. Transcribed Marriage Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Marriage Certificate are blurred or unreadable.


There are some additional documents and it will depend on the type of passport you are holding now. Ito ang mga documents you need to submit also:


For holders of brown passports or passports issued before 01 May 1995:

a. Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page.

b. NSO Birth Certificate. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.


For holders of Machine-Ready Readable Passports (MRRP) or green passports:

a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.


For holders of Machine Readable Passports (MRP):

a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.

b. NSO Birth Certificate to verify complete place of birth. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.

c. Old cancelled passport may be presented in the absence of an NSO Birth Certificate.


For holders of Electronic Passport (E-passport):

a. Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.
