COE Application for Adopted Child Invited by Japanese Foster Parents

Kung meron kayong anak na adopted or inampon ng isang Japanese at gusto nitong papuntahin sa Japan para dito na manirahan kasama nila for good, then this is what you should do para sa inyong COE application.

To apply for COE para sa application ninyo ng visa, ito ang mga sumusunod na documents na dapat ninyong ihanda. Make sure na ang Japanese foster parents ninyo ay willing na papuntahin kayo here at handang mag-prepare ng mga kinakailangang documents para sa COE application ninyo. Actually madali lang ang application at mag-prepare ng mga documents kung willing talaga ang Japanese foster parents ninyo. Ito ang mga documents na kinakilangan:

1. COE Application Form
Known in Japanese as 在留資格認定証明書交付申請書 (ZAIRYUU SHIKAKU NINTEI SYOUMEISYO KOUFU SHINSEISYO). You can get this document sa immigration office or you can download it from immigration homepage. Your Japanese foster parents should fill-up this form and write all necessary information.

2. Pictures (1 Piece)
A picture of the applicant is needed na ilagay sa COE application form, so you need to send this to your Japanese foster parents. Ang size is (4CM[Vertical] x 3CM[Horizontal]), 1 copy only pero mas better na send kayo ng 2 piraso. Write your name at the back of the picture, then idikit ito sa COE application form. Ang picture dapat ay walang suot na sumbrero, walang background at clear ang pagkakuha. Ito ay kinuha dapat within a 3 months period.

3. Family Registration (1 Copy)
Known in Japanese as 戸籍謄本 (KOSEKI TOUHON) is the family registration ng Japanese foster parents ninyo. Makukuha nya ito sa city hall kung saan sya nakatira or naka-register. Kailangan na bago ito na nakuha nya within 3 months period. Pwede rin ang Removal From Family Register Certificate known in Japanese as 除籍謄本 (JOSEKI TOUHON).

4. Birth Certificate (1 Copy)
Kung ang bata ay isinilang dito sa Japan, ang dapat ninyong ipasa para sa document na ito ay Birth Registration Approval Certificate known in Japanese as 出生届受理証明書 (SYUSSEI TODOKEDE JURI SYOUMEISYO). Kung wala kayong makuha nito, pwede naman ang Paternity Acknowledgment Certificate known in Japanese as 認知届受理証明書 (NINCHI TODOKE JURI SYOUMEISYO). Kailangan na bago ito, na nakuha within 3 months period.

Now kung ang bata naman ay isinilang sa Pinas, ang Birth Certificate na nakukuha sa NSO ang kailangan ninyong ipasa. Kung meron itong kasamang Paternity Acknowledgment Certificate, pwede nyo rin itong isama. You should create the Japanese translation version nito at isama ninyo sa pag-submit ng mga documents.

5. Special Adoption Report Approval Certificate (1 Copy)
This is known in Japanese as 特別養子縁組届出受理証明書 (TOKUBETSU YOSHIENGUMI TODOKEDE JURI SYOUMEISYO) na syang magpapatunay na pag-adopt ng Japanese foster parents sa bata. Kung wala kayong makuhang document na ito, pwede rin ang mga document na nanggaling sa Family Court of Japan wherein they are declaring the Japanese as the foster parents of the child. In Japanese, this is 日本の家庭裁判所発行の養子縁組に係る審判書謄本及び確定証明書 (Nihon no katei saibansyo hakkou no yoshiengumi ni kakawaru shinpansyo touhon oyobi kakutei syoumeisyo)

6. Residence Tax & Tax Payment Certificate (1 Copy)
This document ay dapat na ihanda ng Japanese foster parents at ito ay makukuha sa city hall kung san nakatira at naka-register sa Japan. Residence Tax known in Japanese as 住民税の課税（又は非課税） (JUUMINZEI NO KAZEI/HIKAZEI) na nabayaran nya for 1 year at ang Tax Payment Certificate known in Japanese as 納税証明書 (NOUZEI SYOUMEISYO) for a period of 1 year also ay makukuha parehas sa city hall. Sa document na ito makikita ang financial capability ng Japanese foster parents. Kung hindi makakuha ng document na ito ang Japanese foster parents dahil bago pa lamang sya sa lugar at work, pwede syang tumawag sa immigration para sa possible alternative ng document na ito.

7. Guarantee Letter (1 Copy)
Bilang parents, sila ang tatayong guarantor sa pagpunta mo here in Japan. So, this document need to sign by your Japanese foster parents. This is known in Japanese as 身元保証書 (MIMOTO HOSYOUSYO). Your parent need to fill-up this form and put a seal/signature on it.

8. Return Envelope (1 Piece)
Sobre na gagamitin ng immigration para ipadala sa Japanese foster parents ninyo ang result ng COE application. Dapat na nakasulat na dito ang address ng Japanese foster parent ninyo na meron kasamang 392 YEN Stamp (切手, KITTE) na nakadikit na.

9. Others
Dito sa Japan, ang signature ay isinasagawa nila sa paggamit ng tinatawag na HANKO (Seal). Ang Japanese foster parent nyo should have it dahil may mga documents na dapat nyang lagyan ng sign tulad ng Guarantee Letter (MIMOTO HOSYOUSYO). Kung ang application ninyo ay ipapagawa nyo sa ibang tao or third party, kinakailangan nyong gumawa rin ng Authorization Letter na nagpapatunay na sila ang proxy ninyo sa pag-apply ng COE.


IMPORTANT REMINDERS:
1. Ang lahat ng mga documents na hindi nakasulat sa Japanese language ay dapat na ipa-translate into Japanese. Kasama ng original document, you should attached the translated Japanese version of it sa likod nito.

2. As a rule, all documents na pinasa ninyo sa immigration for your COE application ay hindi na ibabalik. Kung meron kayong document na ipapasa at need nyo pa ito at meron kahirapan na makakuha ulit ng original copy, sabihin nyo ito agad sa immigration personnel at the time ng pagpasa ninyo ng mga documents.
